IN-PERSON WELLNESS PROGRAM

Eat smart, move more, feel great, and prevent or delay type 2 diabetes.  This CDC-recognized lifestyle change program was created using proven strategies for reducing the risk of developing type 2 diabetes. Get the support you need to succeed in a program that includes:

  • 16 weekly sessions
  • 6 monthly maintenance sessions
  • $10 vouchers for fresh produce through the course
  • Access to 1:1 mentoring for a full year
  • Group support system
  • Access to programs such as WIC, Tobacco Cessation, Breathe, and a Nutrition Program
  • Incentive items throughout the program as you reach your goals

Introductory Session Zero will take place on  14 June 2023, 4:30pm- 5:30pm. This session introduces you to what you will expect to get out of joining the Diabetes Prevention Program. This will help you see what we do and why we do it as well as give you a little insight of what to expect through the coming year. 

In-person weekly sessions to prevent or delay type 2 diabetes begin 21 June 2023.

Please join us at the TriCounty Health Department for these 45-60 min sessions. Family/friend support is welcome to accompany participants

Classes in Roosevelt City, Duchesne County
