TriCounty Health Department Educators will be offering a FREE in-person wellness program for eligible participants beginning Tuesday, September 10, 2019.
Do you have prediabetes? Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Approximately 84 million American adults—more than 1 out of 3—have prediabetes. Of those with prediabetes, 90% don’t know they have it. Prediabetes puts you at an increased risk of developing type 2 diabetes, heart disease, and stroke.
As part of the National Diabetes Prevention Program, this in-person program will help you learn to eat smart, move more, feel great, and prevent or delay type 2 diabetes. This CDC-recognized lifestyle change program was created using proven strategies that reduce successful participant’s risk by 58%. There is no cost to you. Get the support you need to succeed with a program that includes:
16 weekly sessions
6 monthly maintenance sessions
$10 vouchers for fresh produce through the TriCounty Health Nutrition Program
Access to 1:1 mentoring for a full year
Group support system
Access to programs such as WIC and Breathe – Tobacco Cessation
Incentive items throughout the program as you reach your goals
