Targeted Case Management 

The nurses at TriCounty Health Department have a program called targeted case management that is sponsored by your child’s Medicaid coverage. The best part is it’s free of charge to you.  As nurses, we check your baby’s physical and developmental milestones, answer your child care questions, and link you to local resources that you may not be aware of. Plus, we can provide the same service for all your children through the age of 3.
